But this drug only worked upon repeated use. Probably repeated over quite a long time. In that case, this seems too far fetched... It would make sense, but it doesn't seem to fit with what we know so far. To me at least.

Being given the treatments off island would make it easier. If it was on island, it could have something to do with the water supply from the caves Sun lived there and perhaps, everyone was given the treatment but Sun was the first person to respond and get pregnant.

Actually we don't even know if the drug was successful. It worked to get the subject pregnant, but the male field mouse never made it to term, and we never see whether Juliet's sister actually gives birth.
