Jack decided to do Ben's surgery after seeing Kate and Sawyer together because he just had to get off the island. His heart was broke. Then Kate chose his side over the side that Sawyer went to and they both get off the island together. Of course, based on the FF's so far, they aren't together, though Kate clearly still has a hold on Jack.
