Because, wow, that dude was moving really well for a guy who'd just had his appendix crudely removed. After having my gallbladder removed last summer (in a renowned hospital, and not a tent on the beach), I wasn't able to move comfortably for days, even a couple of weeks.

I think the point is that Jack is tougher than an average patient.

Or, the island decided he'd learned his lesson, and he was allowed to heal fast as is typical there.

Well, when he was running towards Frank's pack, he was clutching his side... I think that's TPTB's way of getting away with Jack doing things he physically probably couldn't be at this point. "Look, he's holding his side and he's in pain! Ergo, continuity!" It's right before the finale, Jack can't really be incapacitated. ;)

Yeah, it healedLocke's crushed spine and cured Rose's cancer. A quick recovery from an appendectomy would be nothing at all.
